A two page fact sheet for the person responsible that defines medical research procedures and provides general points to consider about consenting on behalf of a person with a disability.

Section 42T Certificate: Medical research procedure on a patient who is unable to consent and there is no person responsible to provide consent
A ten page document including general information about the procedural authorisation requirements of a medical research procedure under the Guardianship and Administration Act 1986 for practitioners or supervising practitioners. It also includes a 6-page certificate to be filled out.
Section 42 M Form: Statement of Intent of Registered Practitioner to Provide Medical or Dental Treatment
A two-page form to be completed by the competent person in relation to the refusal of medical trreatment for a current condition.
Section 42 K Notice: Medical or Dental Treatment without the consent of person responsible
A three-page form including introductory material about the notice and how to use it to be completed by the medical or dental practitioner when the patient is unable to consent to medical or dental treatment and there is no person responsible.
Refusal of Treatment Certificate: Competent Person
A two-page certificate to be completed by the competent person in relation to the refusal of medical treatment for a current condition.
Refusal of Treatment Certificate: Agent or Guardian or Incompetent Person
A three-page certificate to be completed by the agent or guardian of the incompetent person in relation to refusal of medical treatment for a current condition.
