Request to the Public Advocate to make a medical treatment decision pursuant to section 63 of the Medical Treatment Planning and Decisions Act 2016 (‘Section 63 Form’)

If a form is submitted after hours, and you believe the request needs to be dealt with prior to business hours, please also call 1300 309 337 to alert the after-hours worker to check for your submission.

Please note: Best practice requires the Public Advocate's delegate to visit the patient before making a medical decision. Where circumstances allow, you should contact OPA well ahead of the time of the proposed treatment.
