Information for service providers (NDIS)

NDIS Service Agreements

As most NDIS service agreements contain matters that go beyond the scope of the authority of a guardian to make decisions concerning access to services, OPA has developed Deeds for both Support Coordination and Miscellaneous Services for NDIS participants who are receiving support services pursuant to an NDIS plan.

These Deeds relate to the provision of support services and include matters that are within the authority of the guardian to agree to on behalf of the represented person.

The Deeds have been updated to allow for them to be filled out and signed electronically via a new online form.

The deeds can still be automatically extended at the end of the original service period where this is agreed to by the provider and the participant.

Electronic signing of deeds during COVID-19 restrictions
